By accessing and using Upliv's care management platform and services, you agree to be bound by these Terms of Service and all applicable laws and regulations. If you do not agree with any of these terms, you are prohibited from using or accessing our services.

Description of Service

Upliv provides a digital platform designed to support individuals with disabilities in managing their care through Ontario's Direct Funding Program. Our services include:

• Care plan management and coordination
• Attendant scheduling and management
• Financial tracking and reporting for Direct Funding
• Communication tools for care teams
• Compliance monitoring and documentation

User Responsibilities

As a user of our platform, you agree to:

• Provide accurate and complete information

Account Termination
